Mckesson launches whole person care management program for texas
health and human services commission
Broomfield, colo.--(business wire)--mckesson today announced that the texas health and human services commission (hhsc) will launch the company’s personal care manager™, a new whole-person care management program for texas medicaid clients, or beneficiaries, through the texas wellness program in early 2011. for the past six years, mckesson has provided care management services to texas medicaid clients. during program years one through five, mckesson’s services generated a total of $40.1 million in net savings for the state. following mckesson’s original six-year hhsc contract, the state put the care management program out for re-bid in 2009; mckesson won this competitive process. mckesson’s personal care manager engages medicaid clients who are at risk or have developed chronic disease, and are identified as being high cost and high risk. nurse care managers teach better ways to manage conditions through personalized care plans and direct contact. better condition management ensures that texas medicaid clients have improved clinical outcomes while hhsc benefits from cost savings derived from fewer hospitalizations and lower healthcare costs as chronic conditions, including behavioral health co-morbidities, are controlled. for medicaid clients with diabetes, mckesson will offer a separate diabetes self-management training program as a component of personal care manager. the diabetes program will be available at multiple medical centers throughout texas. this sub-set of medicaid clients will receive personalized diabetes-specific condition management through small-group and face-to-face meetings with certified diabetes educators. “hhsc asked for a community-based care management program that is high-touch and focuses on the whole person. mckesson’s approach improves medicaid client confidence with disease education, which improves self-management skills,” said naoise colgan, vice president, care management services, mckesson health solutions. to increase client and provider program understanding and participation, mckesson will implement regional care teams to support high-volume clinics. mckesson’s regional care teams work with texas providers and stake holders to successfully integrate the program with provider care plans. the program’s focus on expanding provider engagement improves clinical outcomes and increases cost savings. about mckesson mckesson corporation, currently ranked 14th on the fortune 500, is a healthcare services and information technology company dedicated to helping its customers deliver high-quality healthcare by reducing costs, streamlining processes, and improving the quality and safety of patient care. over the course of its 178-year history, mckesson has grown by providing pharmaceutical and medical-surgical supply management across the spectrum of care; healthcare information technology for hospitals, physicians, homecare and payers; hospital and retail pharmacy automation; and services for manufacturers and payers designed to improve outcomes for patients. for more information, visit http://www.mckesson.com.
